The face lift is probably the most famous procedure in plastic surgery. Actually, infamous may be a better word. Many people think a facelift makes someone look plastic and unnatural. They think it's always noticeable and obvious. Essentially, they think facelifts don't work - but they're wrong.

Today's facelift is more effective and natural-looking than ever. Many patients look as much as five to ten years younger and best of all, they look younger naturally. Instead of looking like they got "lifted," today's patients simply look refreshed, rested and healthy. Friends usually won't know about your facelift unless you tell them. They'll recognize that you look better, but they won't know why. They'll ask if you've been on vacation? Did you lose weight? What exercise program are you on?

With today's new surgical techniques and advancements, facelifts can now last longer than ever before. The surgeon can now tighten up not only the skin but the underlying tissue as well. The average facelift today lasts from five to ten years and follow-up procedures can be performed to keep you looking as good as you can for as long as you can.

The basic idea behind a facelift is to remove excess skin that is loose and sagging on the face and the neck and sometimes to tighten the underlying tissue. If there are bags or pouches around the eyes, eye lid surgery can also be performed at the same time. Sometimes a brow lift is also performed to get the best overall result.

The procedure usually takes from two to five hours. The operation is almost always done on an outpatient basis, with local anesthesia plus sedation. The surgeon begins with an incision along the hairline from the temple down around the ear lobe and to the back of the neck. A small incision may also be made under the chin to remove excess fat. After separating the skin from the muscle and fat underneath, it is then pulled up and back and the excess skin is removed. Sagging muscles and tissues can be tightened and fat deposits removed.

Every effort is made to keep the scarring as minimal as possible. The incisions are closed along natural skin lines and creases, but some scars are unavoidable. Usually they fade and gradually become barely noticeable. Most of the scars are in the hairline or behind the ears. The fine scars at the front of the ears can be easily hidden with makeup if necessary.

After surgery, any pain or discomfort you experience can usually be controlled with medication. The dressings are removed in a few days. Swelling and bruising usually will subside in a week or two. You may experience a certain numbness or tightness in your face or neck which will subside as you heal. Your final result probably won't be realized for at least three weeks. You'll need to limit your exposure to the sun for the first few months after surgery.

Overall, the goal of a facelift is to make you look more like you used to. Together, the tightened skin and tissue lessen and eliminate wrinkles, creases and lines as they restore the younger, natural contours of your face. The way your face used to look five or ten years ago.

It's hard to think of any facial expression that doesn't involve the forehead. Whether you're laughing, crying or even sleeping, your forehead is constantly moving: wrinkling, stretching, expanding and contracting. Unfortunately, all of this activity leaves its mark. Deep forehead wrinkling, and horizontal and vertical frown lines are a common result.

This problem can often be corrected through a forehead lift. One of the biggest benefits of a good forehead lift is that the surgeon can address the underlying tissue and muscles as well as the skin. This helps create a more natural, longer-lasting result. It also has another important benefit. By altering some of the underlying muscles, it can prevent many frown and worry lines from returning.

Every forehead lift also includes an eyebrow lift. Sagging, drooping eyebrows cause heaviness around the eyes and even make them close a bit. They can also make someone look constantly sad, tired and unhappy. When the brow is lifted, the tired, sad look may disappear completely and often the entire face brightens up. After tightening the underlying tissue and altering any muscles that are causing problems, the surgeon removes the excess skin. The incisions are then carefully closed. Every effort is made to keep the scars as minimal as possible. They are hidden along or above the hairline, and are usually very inconspicuous.

The procedure is done on an outpatient basis, with local anesthesia and sedation. The procedure takes from one to two hours. After surgery, you may be up in a day or so. Any pain or discomfort you experience can usually be controlled with medication. The sutures come out within a week, and the bruising and swelling will subside within five to ten days. You can usually return to work in seven to ten days, but you should avoid more strenuous activity for several more weeks. You may experience some numbness, but this almost always subsides within a few weeks.

Especially if your surgeon has altered some muscles, your result should last for years. Five to ten years is the average, and, if you wish, the procedure can be repeated as needed.

In the open brow lift, an incision can be made either in front of the hairline which tends to lower the hairline or behind the hairline near the top of the head, where it tends to raise the hairline. In either case, the incision is quite camouflaged after reforestation in a few months.

Aesthetic eyelid surgery, also called "blepharoplasty," removes the excess fat and wrinkled drooping skin of the upper eyelids that can make you look constantly tired or sad. It also eliminates bags under the eyes and tightens the lower eyelid skin. The result is a brighter, more alert and rested appearance.

Rhynoplasty(Nose)

Surgery of the nose can accomplish a number of things. It can reduce the overall size, improve the tip, remove a bump, narrow or widen the width, or even improve the angle between the nose and the upper lip. Sometimes, breathing problems can be relieved. (in these cases, sometimes the procedure may be covered by insurance.) Generally, any or all of these changes can be made during one operation which usually takes from one to three hours. The procedure is almost always done on an outpatient basis, using local anesthesia with sedation.

Working from inside the nose, the surgeon carefully modifies and reshapes the bones and cartilage. A bump can be reduced or totally eliminated. The sides of the nose can be brought closer together to make the nose narrower. The tip can be made smaller and also lifted to enhance it appearance. When a large, oversized nose is made smaller, sometimes the nostrils also need to be reduced in size. Unbalanced, crooked or curved noses can be straightened and made more symmetrical.

After surgery, a splint is placed on the nose to maintain the new shape and to protect it. Often a nasal pack is inserted in the nose to support the inside. You'll be up and around a day or two after surgery, and any pain you feel can be controlled by medication. Cold compresses on your eyes can reduce swelling and bruising while you keep your head elevated. Within a couple of days the nasal packing comes out. The splint usually comes off within a week.

Bruising around your eyes starts to fade within a couple of days and disappears totally within a few weeks. Most people return to work within a week to ten days and makeup can be used to cover any bruises. You should avoid hitting the nose for at least eight weeks. There will be subtle swelling of the nose which subsides slowly over time. Your final result may not be realized for weeks or even months. Occasionally, additional changes and revisions require additional surgery.

The results will be immediate and dramatic if the nose was very large or if a big hump was removed. But usually, the changes are less dramatic. A slight hump or over-sized tip can be improved without attracting a great deal of attention. Friends may say that they don't see a major difference, but that's okay. Your surgery should improve your appearance without drawing attention to itself.

Remember the idea is not to create a new nose, but rather to improve and enhance the one you have. Your final result will be a permanent improvement which will last the rest of your life.

A chemical peel, or chemosurgery, is a process that minimizes the effects of aging, such as fine lines and wrinkling over the forehead, about the eyelids and cheeks, and around the mouth. It can also be very effective for superficial acne and skin problems associated with chicken pox and uneven pigmentation. The procedure is not a substitute for a face lift, but is often used in conjunction with one. Plastic surgeons have performed this procedure with excellent results. Your doctor can refer you to patients who will discuss their experience with you.

In chemosurgery, a chemical solution is applied to your face. A burning sensation may be experienced when the solution is applied, but should pass quickly as the chemical itself acts as a local anesthetic. Depending on the size of the area to be treated, it takes about an hour or longer and is done on an outpatient basis. Afterwards they may put a mask of adhesive tape over your face which you'll leave on for a day or two. When the peel is done primarily for skin blemishes or pigmentation problems, tape may not be used.

Right away, within 24 hours, a crust or scab will form to protect the new skin. Sometimes this is removed through gentle washing with soap and water followed by the application of ointments, creams or moisturizers. Your surgeon will give you specific instructions. Once the crust has fallen off, the new skin appears deep red. It's similar to a bad sunburn. Gradually, your skin will lighten to a pinkish hue. When the new skin has been regenerated completely after several weeks, it will be a bit lighter in color than it was prior to your procedure. Birthmarks or freckles often disappear. If you have heavy freckles, a line may appear between your chin and neck.

Abnormal sensations, such as itching and tingling, may be experienced as the skin heals. These sensations can be alleviated with cool compresses and appropriate medication. You should be up and around in a day or two after surgery, but should refrain from strenuous activity for two to three weeks.

Since the procedure strips the skin of its protective layer of pigmentation, you should avoid direct sunlight completely for the first three weeks to avoid making your skin tone uneven. Afterwards, when pigmentation returns, any sun exposure should be accompanied with a sunblock.

It will be about three weeks before you can wear light makeup and feel comfortable in public. Most surgeons recommend non-allergenic makeup to avoid irritation to the new skin.

ACNE SCAR Improvement (Dermabrasion)

Scars, fine wrinkles, and other skin surface irregularities can be significantly improved through a procedure known as dermabrasion. This involves the mechanical scraping of the top layers of skin using a high-speed rotary wheel. The procedure essentially polishes the skin, softening the sharp edges of the irregularities, making them much less noticeable.

Dermabrasion may be performed on a small area of the face, such as around the mouth, or the full face. It works best on wide and shallow irregularities. Many times it's combined with other procedures such as facelift, eyelid surgery, or chemical peel. Patients with extensive wrinkles or deep acne scars sometimes require dermabrasion in stages to achieve the optimal result. A facelift may be used to stretch a scar before dermabrasion.

The size of the area being treated determines how long the procedure will take. Small areas may take only minutes, while a full face dermabrasion may take an hour or more. The procedure is conducted on an outpatient basis, with local anesthesia. After surgery, an ointment may be applied to the skin along with a wet bandage. You may feel a tingling along with burning or aching after the anesthesia wears off, but medication can control this.

The following day, the bandage is removed and healing begins. Right away a scab or crust forms as the skin begins to heal and soon, a new skin forms underneath, it falls off. Swelling will subside in about a week or so after surgery.

Your skin will have a pinkish color, and it will be at least a week, probably two, before you can wear light makeup and feel comfortable in public. Afterwards you should avoid direct sunlight, since dermabrasion takes away a layer of pigmentation. Exposure to the sun might result in permanent light or dark spots. Even when your pigmentation returns, you should try to avoid direct sunlight and wear a sunblock or a hat.
